• Joinder Agreement – Complete and sign in front of a Notary.
• Fee Schedule – Print and sign.
• Attorney Self-Funded Pooled Disability Trust Checklist

Include the following:

• Check for the amount to fund the trust made payable to the Trust Company of Virginia.
• Check for the enrollment fee of $1,000 made payable to Commonwealth Community Trust.
